SmithRx 2025 Event Giveaway Official Rules

NO PURCHASE OR PAYMENT NECESSARY TO ENTER OR WIN. MAKING A PURCHASE DOES NOT INCREASE CHANCES OF WINNING. Further eligibility restrictions are contained in the Official Rules below.

  1. DESCRIPTION: The “SmithRx 2025 Event Giveaway Sweepstakes“ (the “Sweepstakes“) will begin on Monday, November 10, 2025, and end at 11:59pm Pacific Time (“PT“) on Tuesday, November 11, 2025 (the “Sweepstakes Period”). The Sponsor of this Sweepstakes is Smith Health, Inc., with its principal address at 300 Brannan Street, Suite 601, San Francisco, CA 94107 (“Smith” or “Sponsor”).

  2. ELIGIBILITY: The Sweepstakes is open only to individuals who are (i) members or beneficiaries of a SmithRx prescription drug benefit plan that is effective as of January 1, 2025, (ii) legal United States residents residing in the forty-eight (48) contiguous United States or Washington, D.C., and (iii) at least eighteen (18) years of age and the age of majority in their state of primary residence at the time of entry (“Entrant”). Employees, owners, shareholders, officers and directors of Sponsor, and each of Sponsor’s agents, representatives, promotion and advertising agencies, together with the Immediate Family Members and/or those living in the same household of such persons (collectively, the “Sweepstakes Entities”), are not eligible to participate in the Sweepstakes.

  3. HOW TO ENTER: To enter, the Entrant must visit /sweepstakes-terms-conditions-event-giveaway, respond to the survey questions, and click “Submit”. Entries must be submitted no later than 11:59 pm PT on Tuesday, November 11, 2025, to constitute an entry. Limit one (1) entry per person for the duration of the Sweepstakes. Participation in the Sweepstakes constitutes Entrant’s agreement to these Official Rules.

  4. WINNER SELECTION AND NOTIFICATION: At the conclusion of the Sweepstakes Period, one (1) Potential Winner will be selected via random drawing. The Potential Winner will be notified on or about Wednesday, November 12, 2025, in the form of an email from the Sponsor. Potential Winners will be required to respond to the Notification and complete a “Prize Acceptance Form” within three (3) business days.

  5. ODDS OF WINNING: Odds of winning depend on the number of eligible entries received.

  6. PRIZES: There is one (1) Prize available: one Yeti cooler with an approximate retail value of Two-hundred Dollars ($200). No cash alternatives or substitution is allowed.

  7. CONDITIONS OF PARTICIPATION: By participating, each Entrant agrees to be bound by these Official Rules and decisions of the Sweepstakes Entities.

  8. RELEASE: By participating in this Sweepstakes and/or by accepting any prize, each Entrant agrees to release and hold harmless the Sweepstakes Entities from and against any and all actions, claims, injury, loss or damage arising from acceptance or use of a Prize.

  9. GOVERNING LAW/JURISDICTION: This Sweepstakes is governed by the laws of the State of California. Participants agree that any disputes shall be resolved before a court located in California.

  10. PRIVACY POLICY: Any personal information supplied by Entrant will be subject to Sponsor’s privacy policy.

  11. WINNERS/OFFICIAL RULES: To obtain the names of the Winners or a copy of these Official Rules, send a request to Smith Health, Inc., 300 Brannan Street, Suite 601, San Francisco, CA 94107. Requests must be received within one month after the end of the Sweepstakes Period.
